OK! The easiest thing you can ever do to help your post secondary grades would be to talk to your profs. I swear to god! I struggled through my first year then I go this wicked idea to go talk to all my profs and honestly, it helped me out more than anything else I could have done. Instantly they feel that you are concerned and care - they see that you are putting in extra effort to connect with them. You'll have some you like and others you like less but it's important to see them all as you may really connect with a few and make friends...
But if time management is the real issue here - make a schedule for yourself. Make a weekly schedule where you have predetermined 1/2 hour slots and assign actions to these slots for every day. The key to success is to limit the amount of non-productive time (free-time) you have. Most schools will have a department you can visit to help you create a schedule or determine better time management practices.
But I would still suggest talking to your profs...hey, maybe you should ask them about a department that can help you with your time management...just a thought and good way to start a conversation and show then you care all at the same time.
